I mean you will always have people say that teams will then fake injuries to get cap relief or some other such conspiracy but it seems fair to offer some sort of proportion back if only a gesture. Say for example, an NFL approved doctor with no team ties has to perform the diagnosis and if someone is truly out for a season then you get a percentage of their cap hit back.
